Please have your insurance card with you at the time of your visit. Copayments and patient responsibility are due at the time of service. Payment should be provided from the guardian that brings the child to the visit, unless prior arrangements have been made. If insurance information is not accurate or coverage has lapsed, you may be responsible for the entire charge at the time of service. We offer a discounted rash for cash pay families.
