2 months	DTaP Hib IPV PCV Rotavirus
4 months	DTaP Hib IPV PCV Rotavirus
6 months	DTaP IPV PCV Rotavirus
6 months + (yearly)	Flu Shot
12 months	Hep A MMR Varicella
15 months	DTaP Hib PCV
18-24 months	Hep A
4 years	DTaP IPV MMR Varicella
10+ years	Meningococcal (Men B) (if local outbreak; absent or damaged spleen; complement deficiency)
11 years	Meningococcal ACWY TdaP
11-12 years	HPV (Gardasil)
15 years	HPV (Gardasil) (if started at 15 yrs, 3 doses- 3rd shot: 6 months after first shot)
16 years	Meningococcal ACWY
16-18 years	Meningococcal (Men B) (Ask us about frequency)
