What Should I Know Before My Visit?

Please arrive at least 15 minutes prior to your scheduled Office Visit. Please bring a copy of your insurance card and photo ID. If you have any current custody agreements, please bring them with you. Copays are due at the time of visit. We do offer a discounted cash price, and payment is due at the time of service. Payment is expected from the guardian that brings the child to the Office Visit. 


Please ensure you are registered for the Patient Portal, developmental forms/screening tools may be emailed to you before your visit, if applicable. If you are a new patient, please print out and bring completed new patient forms to your visit.

We do not accept walk-ins. We operate based on scheduled appointments only. If you need to be seen quickly, we recommend to call the front desk to check availability, or leave a message on the triage line to discuss your specific situation with our experienced nursing staff. 


If you cannot make your appointment we ask that you please reschedule before the appointment time, we prefer 24h notice where possible. If you are more than 10 mins late for your appointment, you may be asked to reschedule or worked in at a later time out of respect to our other patients and their families. Excessive No-Shows could result in dismissal from practice.
